Faculty and Instructors Only:
If you need a video or DVD in our collection for a specific class date, please request the title(s) using the Short Loan Request Form below. Fill out one form for each title needed. (The form is also available at Brookens Library Circulation desk.) Short Loan does not prevent the item from being checked out by others, but it alerts the Circulation Department that the video/DVD is needed on a certain date. Circulation will then give the item a shorter loan period to help insure that it will be back by the needed date.
All the fields on the form are required information.
